As Parent/Guardian of the above-name minor(s), I grant permission for the swimmer(s) to participate in all activities of the Shouse Village Swim Team, a Northern Virginia Swimming League (NVSL) member team. I represent and warrant that my minor child/children participating on the Shouse Village Swim Team are in good health and have no physical condition, ailment or disability which renders them unable to participate in vigorous physical activity. For and in consideration of benefits derived from participation in the Shouse Village Swim Team program, I understand that the risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist. I assume all risks and hazards arising out of, or related to, such participation, including, but not limited to, transportation to and from such activities, and do hereby indemnify, release and hold harmless the Shouse Village Swim Team, its coaches, team representatives, volunteers, property manager, employees and agents, as well as the Northern Virginia Swimming League, from all claims of any kind whatsoever which may arise or hereafter accrue in connection with my child’s/children’s participation in activities of the Shouse Village Swim Team. I further grant permission for first aid to be given to my child/children in an emergency and will be solely responsible for any medical costs which may arise. I AGREE THAT THEY WILL ABIDE BY THE NORTHERN VIRGINIA SWIMMING LEAGUE CODE OF CONDUCT.

The Shouse Village Swim Team is following SafeSport Guidelines.   The NVSL recommends that Parents take the Parent's Guide to Misconduct in Sports (30 Minute Training Video).  It is Free and it is a self study. I agree to allow my swimmer to participate in the Swimming Fundraiser. I indemnify and hold harmless the Shouse Village Swim Team, the Shouse Village Community Association, and the organization for which the swim team is raising funds from all costs, expenses, and liability arising out of my or my child’s participation in this event. I hereby waive all claims for damage or loss to me or my child’s person or property which may be caused by any act, or failure to act, by the Shouse Village Swim Team, Shouse Village Community Association, and the organization for which the swim team is raising funds arising directly or indirectly from my or my child’s participation in this event, and I hereby assume all liability for any loss, damage, or any other liability from such event.

I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing to [email protected], and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.

I do hereby indemnify, release and hold harmless Shouse Village Swim Team, Shouse Village Community Association, its coaches, team representatives, volunteers from all claims, demands, and causes of action arising out of or in connection with any use of said “Materials” including without limitation all claims for invasion of privacy, infringement of my right of publicity, defamation and another other personal and or property rights.
